ATS – Tier II: Equipment Loan

Tier II Equipment Loan is used to REQUEST student-specific light tech items such as: Touch Monitor, Switches, Single message buttons, PECS Books, etc.

 

The intent of the equipment loan is to address student needs by issuing additional equipment to the school. IEP Teams are able to borrow/trial light tech items by submitting an Assistive Technology Service Request. Light tech devices can support areas of need such as communication (e.g., support with picture exchange), writing support, and physical access (for example, a student who is unable to use a standard computer mouse may be able to use a touch monitor, switch with switch interface, or touch pad instead).

An Equipment Loan can be requested for a student via the 2025-2026 Assistive Technology Service Request

STEP 1:

 

Complete a CCF-508 SSD Consent for Special Education Supports from the SEMS website:

STEP 2:

 

Complete and submit the Equipment Loan section of the AT Service Request to request light tech items for the student:

If this has been completed for AT Support for the current school year, another one is not required. If it has not been completed for the current school year:

  • Complete ALL sections

 

  • Select Other and indicate “Assistive Technology”

 

  • Appropriate verbiage includes “Observation, guidance, and/or possible suggestions in the area of Assistive Technology”

 

  • DISTANCE LEARNING PERIOD: During the distance learning time period, please complete the CCF-508 as indicated above and have parent/guardian sign if possible.  If applicable, please indicate “verbal consent” from parent/guardian and indicate the date consent was obtained.  Please refer to the acceptable procedures for documentation of parental consent during distance education; contact your school’s Special Education Instructional Facilitator (SEIF) or building administrator for guidance.

A Tier 2 request can now be utilized to request a student ‘Read&Write for Google’ license or Bookshare consideration.

Upon submission of this form, the AT Itinerant assigned to your school will review the request and may contact you if additional information is required.
