Medicaid School Health Services

The Medicaid School Health Services (SHS) office is available to assist you with all your Medicaid consent and EasyTrac™ needs. For questions,  please call the Medicaid SHS Helpdesk at (702) 799-2899, WAN 0655-2899, or via email at SHS-MedicaidHD@nv.ccsd.net. For additional information, please visit the CCSD Medicaid School Health Services website. We look forward to assisting you.

To request EasyTrac access, save the completed form and forward it to the site/department administrator to be submitted via email to the Medicaid SHS Helpdesk at: SHS-MedicaidHD@nv.ccsd.net.

Student Services Division Data Requests

To ensure all SSD data requests are handled efficiently and accurately, please complete the Data Request Template. All requests should be submitted via email by the SSD Region/Department director. Email the completed form to the SEMS Helpdesk at 0068-sdm-semshd@nv.ccsd.net.
